I can already tell that by making enemies not respawn, it makes the majority of the game not only A LOT more tolerable but easier as well - no more enemies blocking the way beyond a gap that you may get knocked over and get insta-killed. However, despite this, if you still suck at Ninja Gaiden, you still suck at Ninja Gaiden. Don't expect the game to be easy mode after playing just this hack. If you want that, there's other hacks you can apply on top of it.

Combine this with the Restoration + Optional Free Movement hack, and the game will be like it should have been to begin with! Most of the frustration is gone while still allowing one to mess up. Recommended!

Respawning enemies makes platforming a chore and ruins game flow, but with this, no more, you can actually plan jumps properly once the coast is clear, virtually eliminating cheap deaths. Not only recommended, but I'd also go as far as saying it is necessary. I just wish it also included a fix for the infamous 6-4 respawning bug that sends the player way, way further back into the stage than it should.

Literally the first thing I think when I load up games like Ninja Gaiden or Mega Man is why don't the enemies have persistent deaths? I always chalked it up to a limitation of the NES somehow but here we are. Someone do this for Mega Man next...or NG3! What a time to be alive!

It adds backtracking, makes the game have more strategic and satisfying gameplay.

It doesn't break difficulty and gameplay.
